Dog Adoption Application

Before submitting adoption application, please consider the following:

• If the dog is not going to be part of your family, then do not apply.
• You must be at least 21 years of age and employed to adopt a dog. If you live with your parents, they must co-sign the adoption agreement.
• If you have small children, CHS recommends waiting until your children are at least four years of age, unless your children are used to dogs and/or you currently have dog(s). Puppies can be very active.
